- 01HBT7QGXGPT0F36XYYVHC9V76 abstract "Aims: To estimate the association of care-dependency with tooth loss increment in nursing home residents. Methods: The study sample consisted of residents from 34 Flemish nursing homes participating in an oral healthcare programme between 2015 and 2018. Socio-demographic, dental and medical data were extracted from the dental records. Tooth loss increment was determined for dentate residents with at least 11 months follow-up and modelled using negative binomial regression to estimate the effect of level of care-dependency while adjusting for age, number of medications and number of teeth at baseline (admission to the nursing home). The natural log of the duration of the follow-up period was used as an offset variable. The study was approved by the Ghent University Hospital ethics committee (BC-07682). Results: The baseline sample consisted of 1,570 older people (mean age 85.5 years; SD 7.4; range 50 to 107) of whom 765 (56.2%) had high care-dependency. Residents were taking an average of 8.7 different daily medications (SD 3.8; range 0 to 39). Edentulousness at baseline was 36.1% (n = 563). Among the 353 dentate people with >11 months follow-up (mean 22.1 months), the mean number of teeth was 12.4 (SD 7.3); 175 (49.6%) had incident tooth loss, and the mean number of teeth lost during follow-up was 1.8 (SD 3.1, range 0-18). The increment rates for tooth loss among residents with medium and high care-dependency were respectively 1.5 (95% CI 0.71-3.34) and 2.1 (95% CI 0.98-4.40) times greater than for residents with low care-dependency. For every extra tooth at baseline, the adjusted tooth loss increment was greater by 8% ( 95% CI 1.04-1.11). Conclusions: Dentate nursing home residents who are more care-dependent experience more tooth loss over time, and preventive measures should be more intensively focused on that group.".
