Based on the premise that tongue and lip force is affected in individuals with orofacial myofunctional disorders (OMDs), orofacial myofunctional therapy (OMT) programs typically include the strengthening of orofacial muscles through isometric and isotonic exercises. The purpose of this study is to examine whether there is indeed a measurable difference in maximum tongue and lip strength between people with and without OMDs. Methods. Forty-one young adults (mean age 21.1 years, SD 1.67 years, range 17-24 years) participated in this study. Orofacial functions were evaluated using the Orofacial Myofunctional Evaluation with Scores (OMES) protocol. Based on this perceptual evaluation, participants were divided into 3 groups, namely those with physiological orofacial functions (POF; n=14), those with inconsistent orofacial functions (IOF; n=19), and those with consistent OMDs (OMD; n=9). Maximum tongue and lip strength were measured with the Iowa Oral Performance Instrument (IOPI). Results. The OMES scores differed significantly between all three groups (F(2)=12.280; p<0.001). No statistically significant differences in maximum anterior (F(2)=0.903; p=0.414) or posterior (F(2)=1.018; p=0.371) tongue strength, nor in medial (F(2)=0.223; p=0.802), lateral left (F(2)=0.152; p=0.859), or lateral right (F(2)=0.230; p=0.796) lip strength were observed between the three groups. Conclusion. Although orofacial muscle strengthening is commonly part of OMT, current findings suggest the superfluity of these types of exercises in the treatment of OMDs. Re-evaluation of OMT programs, with a focus on awareness and rehabilitation of pathological functions, may be an important strategy going forward.".
